| This may indicate a problem with viewing the form directly through the web browser. The browser may not be capable of displaying the document online due to an incompatibility with Adobe Acrobat Reader. This problem can be resolved by downloading the form and saving it to your hard drive before viewing it.
PC
1.Choose the form or document from the 'Download Documents' box.
2.Right-click on the chosen document link and select 'Save target as...' from the menu of options.
3.Choose a location to save the file, such as the 'Desktop'.
4.When the download is complete, go to the location where you saved it, double-click on the file and it will open within Adobe Acrobat Reader.
Macintosh
1.Choose the form or document from the 'Download Documents' box.
2.Hold down your mouse-click on the chosen document link and select 'Save to disk' from the menu of options.
3.Choose a location to save the file, such as the 'Desktop'.
4.When the download is complete, go to the location where you saved it, double-click on the file and it will open with Adobe Acrobat Reader.
